You can change the appearance of a Contact Us form.
- Log in to your Helpshift instance (or “tenant”) as its administrator.
- On the Helpshift toolbar, click Settings. 
- In the navigation bar, scroll to the APP SETTINGS area. Then, click the name of the container app whose Contact Us form you plan to change. - The App Settings page opens.   
- On the App Settings page, locate the Web Support Portal card.
- On the Web Support Portal card, click  Configure Web Contact Us Form- . - The specified container app’s Customize Contact Us Form page opens.  
- Configure the form settings and values as needed. Then, click  SAVE & PUBLISH- .
